Repeat Prescription Requests
We are unable to take requests for medication on the telephone, however there are a number of ways in which you can order your prescription:
1.) Sign up for online ordering by following the link on the prescriptions page
2.) Send your prescription request to us by post, enclosing a stamped addressed envelope for us to return it to you (please note this option will take longer than the usual two working days, and the Surgery does not accept responsibility for any items that may get lost in the postal system)
3.) Request the required items in writing at the surgery either by completing your request slip (attached to your last prescription) or by completing one of our Manual Prescription Order Forms.
Please note that you do not need to see a GP for access to Podiatry Services. Just complete this
Self-referral form, send it to the address at the top of the form and they will be in touch with you to arrange a suitable appointment.

If you are trying to quit smoking but feel you may benefit from a little extra support, please visit the Quit Ready website at www.quitready.co.uk for more information. They offer a range of online support including text, phone and skype. Face to face appointments are also available.
If you feel that you may benefit for a referral for physiotherapy, this can be completed without needing to see a GP via the Physiotherapy Direct Service.
Telephone 0300 300 0046 and ensure you have your NHS number to hand along with your current address, mobile/home telephone number, next of kin contact number and GP surgery name and address. A list of medications will also be required during the telephone consultation.
Booking staff will take some details from you and will arrange a telephone consultation with a qualified physiotherapist. Advice regarding exercise may be given or an appointment face to face with a Physiotherapist will be arranged where appropriate.
